Safeguarding Record Quality Check
Whether a written record is specific, factual and dated enough to be useful to the person who reads it next.
A safeguarding record is usable when it is dated, uses the child's own words, separates what was observed from what was interpreted, and records what was asked as well as what was said — the four omissions that recur most often in serious case reviews.
Safeguarding Record Quality Check
Frequently asked
What should a safeguarding record contain?
The date and time, what was said in the child's own words, what was asked, what was observed as distinct from what was interpreted, who else was present, the action taken and by whom, and what the child was told would happen next.
Why record the child's exact words?
Because a paraphrase changes meaning and cannot be relied on later. Records that summarise rather than quote are a recurring criticism in reviews of cases where information failed to travel.
When should a record be written?
The same day, and as soon as practicable after the conversation. Detail degrades quickly, and a record written days later is worth substantially less to everyone who reads it.
