SEND Adjustment Tracker

Whether agreed adjustments are actually happening, per pupil, over time — a check on adults rather than on children.

Quick answer

Of fourteen implementation checks, the only one not relying on adults reporting on themselves is asking the pupil whether the support is happening — and it frequently disagrees with the staff view.

Frequently asked

Why do agreed adjustments stop happening?

Because nothing in a school automatically checks. Staff change, cover teachers arrive with no history, and an informal arrangement lasts exactly as long as the person who agreed it. It is a systems problem far more often than an attitude one.

Why do exam arrangements need to match?

In most systems, access arrangements must reflect a pupil's normal way of working. An adjustment that quietly lapses in the classroom can therefore remove an entitlement in an examination, which is discovered far too late.

Who should complete this?

Ideally a SENCO or form tutor with the pupil, because the pupil's answers are the ones that reveal the gap. Staff-only completion consistently produces a higher and less accurate score.
